Job Description

Job Summary:

Medical Records Tech - Cert, applies the appropriate diagnostic and procedural code to patient health records for purposes of document retrieval, analysis and claim processing.

Responsibilities:

  • Abstracts pertinent information from patient records. Assigns the International Classification of Diseases, Clinical Modification (ICD), Current Procedural Terminology (CPT) or Healthcare Common Procedure Coding System (HCPCS) codes, creating Ambulatory Patient Classification (APC) or Diagnosis-Related Group (DRG) assignments.

  • Obtains acceptable productivity/quality rates as defined per coding policy.

  • Queries physicians when code assignments are not straightforward or documentation in the record is inadequate, ambiguous, or unclear for coding purposes.

  • Keeps abreast of and complies with coding guidelines and reimbursement reporting requirements.

  • Other duties as assigned.

  • Abides by the Standards of Ethical Coding as set forth by the American Health Information Management Association (AHIMA) and adheres to Official Coding Guidelines from the American Hospital Association (AHA).

  • Reviews documentation abstracting pertinent data to maintain an accurate data set for the organization and other external reporting such as Health Service Cost Review Commission (HSCRC).

  • Utilizes the appropriate provider documentation, critical thinking skills, coding and clinical knowledge to assign appropriate ICD-10-CM and ICD-10-PCS codes along with the proper POA assignment for proper APR DRG. Queries physicians when code assignments are not straightforward or documentation in the record is inadequate, ambiguous, or unclear for coding purposes.

Qualifications

Licenses/Certifications/Registration:

  • Required Credential(s):

  • One or more of the following:

  • Certified Coding Specialist (CCS) credentialed from the American Health Information Management Association (AHIMA) obtained prior to hire date or job transfer date.

  • Registered Reg Health Info Admnstr credentialed from the American Health Information Management Association (AHIMA) obtained prior to hire date or job transfer date.

  • Registered Reg Health Info Tech credentialed from the American Health Information Management Association (AHIMA) obtained prior to hire date or job transfer date.

Education:

  • HS or Equivalent

  • Required: High School or equivalent, Completion of Medical Terminology, Anatomy and Physiology courses and ICD-9-cm, CPT Coding Courses Preferred: AAS Degree in Health Information Management

Work Experience:

  • Required: One year acute care inpatient coding/outpatient surgery experience. Preferred: Three years acute care inpatient coding/outpatient surgery experience.
